4C Health has released Demonstration Year 1 outcomes from its participation in the Certified Community Behavioral Health Clinic (CCBHC) model.
The CCBHC model is designed to improve access, quality, and coordination of behavioral health services through evidence-based, person-centered, and data-driven care. During Calendar Year 2025, 4C Health exceeded or met many state-level benchmarks, demonstrating meaningful progress in early identification, timely access to services, and integrated whole-person care.
Strengthening the Rural Behavioral Health Workforce
As part of its CCBHC Demonstration Year 1 efforts, 4C Health made significant investments in rural workforce development to expand access to high-quality behavioral health care. Since July 2024, the organization has added 116 new positions, substantially increasing service capacity across the rural communities it serves. In Calendar Year 2025, 4C Health also trained 41 students, including 23 master’s-level therapy students, helping build a strong and sustainable pipeline of future behavioral health professionals.
These efforts have contributed to a 46% reduction in staff turnover, reflecting improved workforce stability, retention, and organizational sustainability. Workforce development under the CCBHC model is not only a driver of clinical success—it represents a critical investment in rural community stability, economic opportunity, and long-term access to care.
Dr. Carrie Cadwell, President/CEO of 4C Health states, “The Certified Community Behavioral Health Center model is not just a vastly improved care operating system, it is a local, rural driver of workforce development and then in turn economic development. The CMHC operating system to date simply was and is incapable of achieving the quadruple aim of quality care, high level accessibility, workforce development, and local community economic development. When you are talking about ‘well’ rural communities, you need all of it. CCBHC in Indiana is rural health transformation!”
Key Performance Highlights
During Demonstration Year 1, 4C Health significantly exceeded access standards for timely behavioral health services. Individuals seeking care received an initial evaluation within an average of 2 days, far surpassing the CCBHC goal of less than 10 days. Additionally, clients began ongoing services within an average of 12 days, exceeding the benchmark of less than 17 days. These outcomes demonstrate the effectiveness of the CCBHC model in reducing wait times, removing barriers to care, and ensuring individuals and families are connected to needed services quickly—an especially critical achievement in rural communities.
“In a CCBHC, quality measures go beyond numbers—they show us that care is working, and people are getting what they need, when they need it. Over the past year, we’ve seen our metrics improve and have used them to drive meaningful system changes. These measures have helped us expand access, strengthen services, and break down barriers to care”, said Liz Day, Chief of Staff of 4C Health.
