Community Fairbanks Behavioral Health transition of services to 4C Health in Howard, Tipton counties

Community Fairbanks Behavioral Health has announced a planned transition in service designation for Howard and Tipton counties.

Effective July 1, 2026, 4C Health will assume responsibility as the designated Community Mental Health Center (CMHC) for behavioral health services in both counties.

Both organizations are working closely together to provide a seamless and supportive handoff for individuals, families, and community partners. Community Fairbanks Behavioral Health and 4C Health will continue working in partnership to support the community’s behavioral health and broader healthcare needs, ensuring coordinated, person-centered care.

Dr. Carrie Cadwell, CEO of 4C Health states, “It is with deep gratitude and serious commitment that 4C Health enters this next level of dedication to Howard and Tipton counties. We are already the Certified Community Behavioral Health Center for both counties. Adding the additional certification as the Community Mental Health Center just elevates all our existing services for these communities and creates opportunity for expanded services.”

4C Health brings extensive experience serving rural and community-based populations, with a strong focus on accessibility, innovation, and whole-person care. The organization has proudly served both communities for more than a decade, providing highly accessible, high-quality behavioral health and substance use services. As the designated CMHC, 4C Health will continue to ensure comprehensive, community-based mental health and addiction services are available to all who need them. The organization is actively preparing to expand services and deepen community partnerships in Howard and Tipton Counties.

“The transition reflects a collaborative effort to ensure long-term sustainability of needed mental health services in our communities,” said George Hurd, Vice President of Community Fairbanks Behavioral Health. “As the Certified Community Behavioral Health Center for Tipton and Howard counties, 4C is the appropriate provider to take on the Community Mental Health Center designation and its associated community-based services. Community Fairbanks will maintain continuity of care during this transition, and we remain committed to providing quality outpatient mental health services at out clinics in Tipton and Kokomo.”
