4C Health is reporting continued progress through its federally funded Rural Enhancement and Access for Consumers Experiencing Homelessness (REACH) Grant, supported by the Substance Abuse and Mental Health Services Administration (SAMHSA).

The project focuses on connecting individuals experiencing homelessness with outreach, housing

resources, treatment, and recovery support across Cass, Miami, and Howard counties. Through September 16, 2026, the REACH project has completed 729 outreach contacts. Of those served through outreach efforts, 297 individuals received at least two outreach contacts, reflecting the continued engagement often needed to build trust, identify individual needs, and connect people with appropriate support.

Since the start of the grant, 385 unique individuals have been served. Among those individuals, 69 have achieved housing outcomes through permanent housing placements or entry into the REACH recovery housing voucher program. Additionally, 154 individuals have entered treatment services of some kind.

These outcomes reflect the project’s commitment to meeting people where they are and helping them navigate barriers to housing, behavioral health care, substance use treatment, and recovery support.

“Homelessness in rural communities can be difficult to see, and even more difficult to navigate without the right connections and support,” said Dr. Carrie Cadwell, President/CEO of 4C Health. “These outcomes represent individuals who have been reached, relationships that have been built, and opportunities for people to connect with housing and treatment services. We recognize that progress looks different for every person, and we remain committed to working alongside individuals and community partners to help people move toward greater stability and recovery.”

Funded by the Substance Abuse and Mental Health Services Administration (SAMHSA), the GBHI project supports expanded outreach, treatment access, recovery-oriented services, and connections to housing resources for individuals experiencing homelessness who may also be living with substance use disorders or co-occurring mental health conditions.

Through its REACH (Rural Enhancement and Access for Consumers Experiencing Homelessness) initiative, 4C Health focuses on addressing the unique challenges of rural homelessness across Cass, Miami, and Howard counties. Outreach teams work to engage individuals in their communities, connect them with available resources, and help them navigate pathways to behavioral health care, treatment, recovery support, and housing.

The work recognizes that connecting someone with services is often not a single interaction.

Consistent outreach and relationship-building can be essential to helping individuals overcome barriers, engage in treatment, and identify options that support long-term stability.

“Every outreach contact is an opportunity to build trust and help someone take a step toward the support they need,” said Alanna Lease, Director of Adult Intensive and Residential Services.

“Our team works to meet individuals where they are, understand their circumstances, and connect them with resources that can make a meaningful difference. These numbers reflect the importance of persistence, collaboration, and individualized support in addressing homelessness in our communities.”

4C Health continues to collaborate with local organizations and community partners to strengthen the network of support available to individuals' experiencing homelessness and housing instability. By connecting outreach with treatment, recovery services, and housing resources, the organization aims to reduce barriers to care and help individuals move toward greater health, independence, and stability.

While the reported outcomes reflect meaningful progress, the need for accessible services and housing resources remains. 4C Health will continue building relationships, strengthening community partnerships, and connecting individuals with services that support their individual goals.
