 
                            
4C Health is excited to announce the continued expansion of its primary care services, furthering its commitment to delivering whole health solutions to the communities it serves. Initially launched in fall 2024, the primary care program has steadily grown and is now actively serving individuals and families across Fulton, Miami and Cass counties.
Supporting this growth, 4C Health has built a strong team of four dedicated primary care providers, nurse practitioners Olivia Hayes, Mark Lentz, Casey Hill and most recently Stacie Stutzman.
"I am passionate about providing care that focuses on addressing both the physical and mental health needs of patients," Stutzman said. "I am excited to be part of this team and enthusiastic about contributing my knowledge and skills to this effort."
4C Health remains focused on expanding and strengthening its primary care services, in both youth and adult populations, ensuring that individuals in the region have access to comprehensive, integrated health care that supports both their physical and mental well-being.
As 4C Health continues to grow its primary care division, it is building a robust infrastructure designed to deliver optimal outcomes for clients and patients throughout the region.
