MHP awarded American heart Association Outpatient Program Achievement Awards

MHP Primary & Specialty Care: Family & Internal Medicine has received all three of the American Heart Association’s outpatient program achievement awards.

The recognition highlights MHP’s commitment to reducing the risk of heart disease and stroke by improving management of high cholesterol, Type 2 diabetes, and blood pressure. These awards honor organizations that follow the latest evidence- and science-based care guidelines.

Nearly half of all adults in the U.S. live with some form of cardiovascular disease, including heart attack, stroke, or heart failure. High cholesterol, Type 2 diabetes, and uncontrolled high blood pressure are leading risk factors. With timely diagnosis, science-based treatment, and patient education, these conditions can be successfully managed.

“We are proud to be recognized by the American Heart Association for our focus on Type 2 diabetes, cholesterol, and high blood pressure management and care,” said Dr. Emily Ann Andaya, Medical Director of the MHP Cardiometabolic Program. “MHP is a small rural healthcare organization with a big voice and a bold vision. We believe that when we put our patients at the center, everything else will follow. These programs allow us to apply the latest science-backed guidelines to improve the lives of our patients.”

The American Heart Association’s outpatient achievement award programs provide health care organizations nationwide with resources to ensure patient care is aligned with the latest evidence-based guidelines. Cholesterol™, and Target: Type 2 Diabetes℠, MHP was recognized for demonstrating a strong commitment to improving patient risk factor management.
