4C Health continues to proudly commemorate its 50th anniversary in 2025, marking five decades of dedicated behavioral health services to north central rural Indiana.
What began in 1975 with just a few committed community members has grown into a thriving agency with more than 500 associates, across seven counties, united in their mission to improve health and wellness throughout the region.

As part of its anniversary celebrations, 4C Health is recognizing exceptional individuals who have made significant contributions to behavioral health. This year, the agency launched the Behavioral Health Champions of 2025 awards, honoring leaders and advocates who have gone above and beyond in support of mental health and substance use prevention and treatment.
Among the honorees, Representative Ethan Manning was named Legislative Champion of the Year. Representative Manning has consistently been a strong and compassionate voice for rural communities, advocating for greater access to healthcare in rural areas, nursing workforce supports, and the viability of small and rural hospitals.
Dr. Carrie Cadwell, CEO of 4C Health, had this to say, “We honor Representative Manning as a Behavioral Health Champion because when you are a consistent advocate for rural communities’ health and well-being that translates into support for whole health, support for mental well-being.
There is no health without mental health. We are deeply appreciative of his legislative contributions and support for all aspects of rural healthcare. 4C Health being one of those rural healthcare providers."
