4C Health is proud to announce new partnerships with Kokomo Rescue Mission and Gilead House to provide on-site behavioral health services for individuals experiencing homelessness and housing instability in Howard County and surrounding communities.
Through these partnerships, 4C Health clinicians will provide direct behavioral health support within both organizations’ facilities, helping individuals access care in the same locations where they already receive shelter, meals, and supportive services. The initiative aims to remove barriers to care and ensure individuals facing housing instability can receive timely mental health and substance use support.
This collaboration also represents an extension of outreach efforts that began through the Grants for the Benefit of Homeless Individuals (GBHI) granted from SAMSHA over a year ago, which focused on expanding access to behavioral health services for underserved and
vulnerable populations. Building on that foundation, 4C Health continues to strengthen community partnerships that improve access to care for individuals experiencing homelessness.
“Meeting people where they are is a critical part of improving access to behavioral health care,” said Alanna Lease, Director of Adult Intensive Services from 4C Health. “These partnerships allow us to continue the momentum created through the GBHI initiative while expanding our ability to connect individuals with the support and resources they need.”
Through the partnership, individuals served by Kokomo Rescue Mission and Gilead House will have access to behavioral health screenings, counseling services, and connections to ongoing treatment and support through 4C Health. Staff at both organizations will work
collaboratively with 4C Health clinicians to identify individuals who may benefit from additional behavioral health services.
Leadership at Kokomo Rescue Mission and Gilead House emphasized the importance of integrating mental health and substance use services into the broader support network for individuals experiencing homelessness.
By embedding behavioral health professionals directly within trusted community organizations, 4C Health and its partners aim to increase access to care, improve outcomes, and support long-term stability for individuals across Howard County and neighboring communities.
