4C Health has added primary care to its service offerings as part of its mission to provide high quality, whole healthcare to rural community residents of Cass, Miami and Fulton counties. Blending behavioral health services with primary care creates access, quality and convenience for residents versus traditional models that require patients to travel to multiple location for multiple different healthcare services.
Primary care services began in Logansport in December 2023 and in Rochester in August 2024. These services are now expanded to Peru, effective October 2024.
