MHP Primary & Specialty Care: Family & Internal Medicine has received all three of the American Heart Association’s outpatient program achievement awards.
The recognition highlights MHP’s commitment to reducing the risk of heart disease and stroke by improving management of high cholesterol, Type 2 diabetes, and blood pressure. These awards honor organizations that follow the latest evidence- and science-based care guidelines.
Nearly half of all adults in the U.S. live with some form of cardiovascular disease, including heart attack, stroke, or heart failure. High cholesterol, Type 2 diabetes, and uncontrolled high blood pressure are leading risk factors. With timely diagnosis, science-based treatment, and patient education, these conditions can be successfully managed.
